Position Description

Program Director for MI Choice Waiver

This program covers 10 Northern Michigan counties and serves the elderly and disabled population in the community as well as the institutional Nursing Home setting. The program is centered on consumer choice and living as independently as possible with supportive services to accomplish participant identified goals and objectives. The Program Director works to ensure that quality case management services are provided, ensuring quality and safe delivery of in home supportive care services.

Position Functions:

  • Serve as the Director of the MI Choice Home and Community Based Services for the Elderly and Disabled Medicaid Waiver Program.
  • Carry out supervisory responsibilities which includes supervisory sessions, performance monitoring and appraisals, professional development planning and address disciplinary issues when necessary.
  • Knowledge and understanding of basic human resources policies, procedures, laws and union contract requirements.
  • Ensure service and residential provider contracts are in place and meet the standards established by MDHHS MI Choice Home and Community Based Services for the Elderly and Disabled (HCBS/ED) Medicaid Waiver Contract.
  • Ensure providers comply with all performance standards identified in contracts.
  • Manage all MI CHOICE Waiver provider contract negotiations
  • Responsible for budgeting and fiscal monitoring for the MI Choice HCBS/ED Medicaid Waiver Program.
  • Responsible for the overall quality plan of the MI CHOICE Waiver Program.
  • Responsible for monitoring of participant ongoing eligibility criteria, both financial and medical eligibility. Maintain appropriate tracking as mandated by MDHHS MI CHOICE Waiver contract requirements.
  • Remain abreast/search for and introduce when appropriate best practice or new models of care coordination. Stay informed of changes occurring within the Home and Community Based services platform and work to inform and encourage NLCMHA and NHCM to be adaptive to changes and enhance stability of the program.
  • Maintain liaison status with MDHHS, Community Network agencies, and other departments affiliated with the intricacies of Home and Community based services and advocates for those served through this program.

MINIMUM EDUCATION AND EXPERIENCE:

  • Licensed Psychologist, Licensed Masters Social Worker or BSN Registered Nurse.
  • Minimum of five (5) years of experience in direct provision of community services in a community service program. Three (3) years of clinical supervision. Three (3) years managed care experience 

LICENSING:

  • Licensure with State of Michigan as a Licensed Psychologist, Licensed Masters Social Worker, or BSN Registered Nurse.
